New Patient Appointments

Before your first appointment with us, please download the forms below and then print and complete them. Please bring the completed forms to your first appointment. Also, be sure to complete the Phreesia pre-registration received via either text or email. This will be sent to you a few days before your appointment.

Your Initial Visit

Please arrive thirty minutes before your appointment so that we can review your information and create your chart. Bring a list of all of your medications and your insurance card(s) with you. If possible, please bring your medical records with you. Please be advised that we do not accept personal checks on your first visit. Cash, credit cards (VISA, Master Card, and Discover), and money orders are accepted.
